Booking your surgical consultation

A $350 consultation fee is collected to secure your surgical consultation appointment with Dr. Purewal. If you decide to book your surgery within seven days of your consultation, the consultation fee will be applied towards your surgery fee.

Booking your facial rejuvenation consultation

A $350 consultation fee is collected to secure your facial rejuvenation consultation appointment with Dr. Purewal. If you decide to book a service on the day of your consultation, the consultation fee will be applied towards your service fee.

To schedule a surgery

1. A $2,500 nonrefundable booking fee is required to secure a surgery date. The nonrefundable deposit can only be applied towards surgery fees.

2. The total surgery fee must be paid within two weeks of your surgery date. If surgery is scheduled within less than two weeks of your surgery date, the total surgery fee must be paid when scheduling.

3. If the final surgery payment is not collected within two weeks of the surgery, the surgery appointment will be canceled and any fees collected are forfeited.

4. Surgical fee is quoted or honored for one month from the date of quotation.

5. Anesthesia and facility fees are subject to change per Surgical center as well as medical clearance requirements.

Surgery cancellation / rescheduling policy

1. Should it become necessary to cancel or reschedule your surgery, you must provide a minimum of four weeks notice.

2. The first reschedule will occur a $500 scheduling fee. If you reschedule more than one time, there will be a $1000 rescheduling fee.

3. In addition, any new surgical pricing will be applied at the time of change

4. If surgery is canceled within two weeks prior of surgery 50% of all fees collected are forfeited.

5. If surgery is canceled within one week prior to surgery 100% of all these collected are forfeited.

To schedule a procedure (non-surgical)

1. A $1000 nonrefundable booking fee is required to secure a procedure date. The nonrefundable booking fee can only be applied toward the booked procedure.

2. Procedure fee is quoted or honored for one month from the date of quotation.

3. Procedure fee is quoted or honored for one month from the date of quotation.

Procedure cancellation / rescheduling policy

1. Should it become necessary to cancel or reschedule your procedure you must provide a minimum of five business days notice.

2. Two if you cancel or reschedule your procedure in less than five business days a $150 cancellation fee will be charged.

3. the cancellation fee will have to be paid in advance before scheduling a new procedure appointment.

4. any new procedure pricing at the time of rescheduling would be applied.

Procedure package policy

1. The entire package balance must be paid on the first procedure day.

2. The entire package fee is nonrefundable and can be only applied toward the booked procedure. 

All fees are paid by credit card, check, money order or cash only. Personal checks are not accepted for first time patients for same day services. We do offer convenient, budget friendly, financing options such as Care Credit and Cherry through Alle.
